  • Key Responsibilities
  • The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals.
  • Coordinates to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms.
  • Must be detail-oriented, possess the ability to multitask, and be open to cross-functional training in all departments.
  • Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel.
  • Pulls referrals off platforms to accept/decline, Reviews documents, confirms, and processes the referral through Intake using Pinnacle’s EMR.
  • Responds to Care Coordinator inquiries regarding referral status promptly.
  • Responds to all inquiries with a resolution to concerns and or discrepancies.
  • Coordinates and communicates with the appropriate team regarding any changes or updates to the patient’s payer source on time.
  • Meet productivity goals as assigned by the direct supervisor.
  • Complies with HIPAA regulations on all accounts.
  • All other responsibilities assigned by leadership.
  • Qualifications
  • Must have a GED or High School Diploma, Associate's, or Bachelor’s Degree in a related field preferred.
  • Ability to manage conflict, stress, and multiple simultaneous work demands in an effective, professional, positive manner. Analyzing and reconciling complex issues and problems timely.
  • Constructively working when faced with high workloads and deadlines.
  • Follows directions and respectfully requests clarification when needed.
  • Identifies needs and opportunities and presents them to the supervisor with recommended solutions.
  • Ability to work independently with minimal supervision while collaborating with other team members, build and maintain relationships, and be results-driven.
  • Prepare and analyze appropriate reports and other business correspondence.
  • Always maintain a positive demeanor while contributing to a climate of collaboration and cooperation.
  • Possesses excellent written, verbal, and listening communication skills.
  • Foster two-way communication.
  • Address customer concerns in a courteous and timely manner.
  • Strong organizational and time management skills.
  • Working knowledge of EMR database: Allscripts, Navi, Epic NCH, PowerChart.
  • Proficient with Microsoft Word, Excel, and Outlook software.
  • Possesses good customer relations skills and a commitment to providing quality service.
  • Must possess excellent prioritization and time management skills.
  • Must be able to communicate effectively and clearly.
